Job summary

Partners Health Management in North Carolina seeks an Innovations Care Manager (QP) to provide Tailored Care Management for members with intellectual and developmental disabilities in the NC Innovations waiver. You will conduct comprehensive assessments, create care plans, promote wellness, and coordinate waiver services across providers.

Travel to communities is essential; the role supports I/DD individuals in community settings.

Qualifications

• Knowledge of assessment and treatment of I/DD needs
• Ability to develop strong, person-centered plans
• Proficient in Microsoft Office and data tracking tools
• Experience with LTSS/HCBS coordination is preferred

Responsibilities

• Provide comprehensive care management across primary, behavioral, and specialty health care
• Develop and update the ISP and care plans within required timelines
• Educate members and caregivers on budget development and waiver requirements
• Monitor services for compliance with waiver standards and Medicaid regulations
• Coordinate transitions and referrals to community supports
