Job summary
Trillium Health Resources is seeking a Care Manager – IDD to join our team. This remote role requires licensure and NC practice eligibility, overseeing complex care coordination for members with intellectual/developmental disabilities.
You will gather information, educate members on rights, perform assessments, and update plans as life circumstances evolve, collaborating with a multidisciplinary team to ensure waiver adherence and quality outcomes.
Qualifications
- Fully licensed by NC governing board regulating Human Services professions.
- RN license in NC with 4 years MH/DD/SA experience.
- Master's in human services with 1 year MH/DD/SA experience.
- Bachelor's in human services with 2 years MH/DD/SA experience.
- Bachelor's in non-human-services with 4 years MH/DD/SA experience.
- Must meet LTSS/HCBS coordination experience if serving LTSS caseload.
- Valid driver's license required.
- Must reside within Trillium's catchment of North Carolina.
Responsibilities
- Provide complex care coordination and monitoring for members with IDD.
- Coordinate care with multi-disciplinary teams and ensure waiver compliance.
- Conduct discovery and assessments for planning processes.
- Update plans and assessments as member life changes occur.
