Job summary

Trillium Health Resources is seeking an Intellectual/Developmental Disability (I/DD) Care Manager to provide complex care coordination for members with IDD who may have secondary MH/SA needs or complex medical/physical health needs. The role requires proactive assessment, monitoring and cross-disciplinary collaboration to ensure quality support and waiver adherence.

On a typical day, you will use person-centered planning, educate members on rights and responsibilities, complete discovery

Qualifications

• Fully licensed by the NC governing board OR RN licensed in NC with MH/DD/SA experience.
• Master’s degree in a human service field with 1 year MH/DD/SA experience.
• Bachelor’s degree in a human service field with 2 years MH/DD/SA experience.
• Bachelor’s degree in a non-human services field with 4 years MH/DD/SA experience.
• LTSS/HCBS coordination experience preferred.
• Valid driver’s license required.
• Must reside within Trillium’s Catchment of North Carolina counties and be able to travel within catchment.

Responsibilities

• Utilize person-centered planning methods to gather information and address member needs.
• Provide education and support to members about rights and responsibilities.
• Complete discovery and assessment activities as part of planning.
• Ensure assessments and plans are updated as life circumstances change.
