Job summary

Trillium Health Resources seeks an Innovations Waiver Care Manager to perform complex care coordination for individuals with Intellectual/Developmental Disabilities, possibly with mental health or substance use needs. You will assess, monitor, and coordinate care across teams to maintain waiver compliance and high-quality support.

The role emphasizes person-centered planning, education for members on rights and services, and coordination with providers to ensure objective, comprehensive care.

Qualifications

• Must meet QP status or equivalent licensure as required.
• Licensed RN or licensed human services professional with relevant experience.
• Bachelor’s or Master’s degree in a human service field with MH/DD/SA experience.
• Two years of LTSS/HCBS coordination experience preferred.

Responsibilities

• Provide complex care coordination and monitoring for Innovations Waiver participants.
• Engage in multi-disciplinary collaboration to ensure waiver requirements are met.
• Educate and support members on available Medicaid and state-funded services.
• Assist members in choosing service providers with objective coordination.

Education

Master’s degree in a human service field

Bachelor’s degree in a human service field

Licensed professional (QP status / NC RN license)

Four years MH/DD/SA experience (if RN or other licensure)

Two years LTSS/HCBS coordination experience

Valid driver’s license
