Job summary

Epiphany Family Services, LLC seeks a Care Manager Extender to support Tailored Care Management for Behavioral Health Tailored Plan beneficiaries. Qualifications

• Must be at least 18 years old with a high school diploma or equivalent.
• Trained in Tailored Care Management as described in the document.
• Supervised by a TCM Care Manager and meet one of the listed experience criteria.

Responsibilities

• Share personal story to assist/support the member.
• Perform general outreach, engagement, and follow-up with members.
• Coordinate services/appointments and arrange transportation.
• Engage in health promotion activities and knowledge sharing.
• Share information with care team about member circumstances.
• Provide and track referrals and assist with community resources.
• Participate in case conferences.
• Support in assessing and addressing unmet health-related resource needs.
• Maintain regular communication with Care Manager and Care Manager Supervisor to reduce unnecessary outreach.
