Job summary

Portland Opportunities Industrialization Center (POIC) seeks an Elevate Care Manager to join the CHI Elevate for Black Men team in Portland, OR. The role focuses on intensive case management for African American men 17–25 at high risk, with culturally informed supports and crisis intervention.

The position requires flexibility for evenings/weekends, collaboration with schools and community partners, and a commitment to anti-racist, equitable service delivery.

Qualifications

• Bachelor’s degree required or equivalent and 3+ years demonstrated case management experience.
• Master’s degree preferred.
• Experience working with communities of color, especially African American young adult men and their families.
• Experience with diverse age groups from children to older adults.
• Ability to assess risk factors and develop interventions.
• Knowledge of mental health, substance use, and behavior issues.
• Ability to facilitate and teach skill-building groups for adults.
• Flexible schedule including weekends and evenings.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess and implement strength-based success plans grounded in culturally specific practices.
• Facilitate the Black Male Success curriculum focusing on skills, resources, relationships, and barriers.
• Collaborate with schools, community organizations, and justice agencies.
• Provide day-to-day social casework, intake, assessment, and engagement activities.
• Coordinate pro-social activities for adults and families.
• Maintain documentation in CHI policies and data systems.
• Respond to crises and risk incidents within required timeframes.
• Perform other duties as assigned.
