Job summary

New Narrative in Portland, OR seeks an ICM Clinical Case Manager to deliver community-based interventions for high-need individuals on the Intensive Case Management team in Multnomah County, using trauma-informed and DEI-focused approaches.

You will work alongside a Mental Health Clinician to implement and document service plans, coordinate care, connect clients to benefits, transportation, and community supports, and maintain timely documentation and crisis prevention.

Qualifications

• Qualified Mental Health Associate with a Bachelors Degree in social work or related field is required.
• Progress toward obtaining CADC I certification or willingness to attend classes and receive supervision toward certification.
• Spanish/English desirable.
• A minimum of two years of experience in social work or related field preferred.
• Ability to obtain Oregon Driver’s license within 30 days of hire.

Responsibilities

• Case Management: collaborate with Mental Health Clinician and participant to create and implement the participant’s service plan.
• Wrap around services through interdisciplinary care to meet basic needs and promote safety and independence.
• Coordinate with medical providers and assist with meeting IADL needs.
• Provide transportation using company vehicle to appointments and services.
• Monitor, coordinate, and document all agency services and outcomes.
• Develop relationships with providers to facilitate service provision and housing stability.
• Maintain regular contact with consumers and identified supports; coordinate with external providers.
