Job summary

Bridges to Change is seeking a full-time Case Manager I in Multnomah County, OR to provide peer support and case management for female-identifying participants, helping with community integration and access to resources. You will implement service plans under supervision while modeling recovery principles and maintaining strong ethics.

The role emphasizes trauma-informed engagement, documentation, and collaboration with partners to support participant goals in a diverse, dynamic environment.

Qualifications

  • PWS certification required within 90 days of hire.
  • Self-identifies as a peer with lived experience in mental health or related challenges.
  • Minimum 1 year of continuous sobriety for those with addiction-related lived experience.
  • ORCHARDS BCU background check clearance required.
  • Valid driver's license with vehicle and active registration and liability insurance.
  • Ability to conduct directly observed urinalysis, gender-specific requirement.

Responsibilities

  • Deliver peer support services and case management, dedicating at least 50% of scheduled time to direct client services.
  • Assist participants from diverse backgrounds to access services and resources.
  • Implement individual service plan goals.
  • Promote community integration by supporting recovery models.
  • Develop recovery skills and harm-reduction strategies.
  • Provide trauma-informed, motivational interviewing and mentorship.
  • Provide transportation assistance for appointments.
  • Document services and progress within 24–72 hours.
  • Respond to crises with trauma-informed approaches and Narcan when needed.
  • Collaborate with community partners for care coordination.
  • Participate in supervision, meetings, and trainings.
  • Adhere to policies, confidentiality and ethics.
