Job summary

New Narrative seeks an on-site Clinical Case Manager in Portland, OR, to support high-need individuals on the Intensive Case Management team. You’ll work in a trauma-informed, DEI-focused role, connecting clients with resources, housing support, and community-based services.

Responsibilities include developing service plans, coordinating benefits, and advocating for participants. 60–75% of time is community-based with some office tasks, using a company vehicle when needed.

Qualifications

• Qualified Mental Health Associate with a Bachelor’s Degree in social work or related field is required.
• Progress toward obtaining CADC I certification or willingness to attend classes and receive supervision toward certification.
• Spanish/English desirable.

Responsibilities

• Create and implement the participant’s service plan with the ICM team.
• Coordinate wrap-around services and link to benefits (SSI/SSDI, SNAP).
• Advocate for and with the participant; provide transportation for appointments.
• Monitor, coordinate, and document all agency services with external providers.
• Collaborate with Mental Health Clinicians to apply appropriate interventions and skills training.
• In crisis, de-escalate, connect to emergency services, and document events within 24 hours.
