Job summary

Bridges to Change in Hillsboro, OR seeks a full-time Case Manager I to support male-identifying participants with peer-centered, recovery-focused services. You will help with integration into the community, skills development, and access to essential resources under supervision.

The role emphasizes recovery principles, ethics, and consistent participant support. It requires certification within 90 days, a valid driver’s license, and trauma-informed, culturally responsive engagement.

Qualifications

• PWS certification required within 90 days of hire
• Lived experience in mental health/addiction or related challenges relevant to population served
• Minimum 1 year of continuous sobriety for those with addiction-related lived experience
• Valid driver’s license with active registration and insurance
• Ability to conduct directly observed urinalysis while maintaining safety and privacy

Responsibilities

• Deliver peer support services and case management, dedicating 50%+ of time to direct client services
• Support diverse participants by recognizing unique needs and assisting access to resources
• Implement individual service plan goals as directed by Case Manager II
• Promote community integration by building support networks and recovery models
• Provide trauma-informed guidance, de-escalation, and crisis support as needed
• Document services and progress notes within 24–72 hours
• Collaborate with community partners to coordinate care and services
