Qualifications

• Proficient with PC-based software programs.
• Strong organizational and interpersonal skills.

Responsibilities

• Practice care management within licensure scope.
• Coordinate care for a high-risk member population.
• Develop and implement care plans with member, family, PCP, and care team.
• Communicate with providers to facilitate needed care.
• Document all activities in Care Management software.
