Job summary

New Narrative in Portland, OR, seeks an ICM Clinical Case Manager to deliver community-based interventions for high-need adults. You will work with the Integrated Dual Disorders team, implement service plans, coordinate with providers, and advocate for participants to improve housing stability and wellness.

Travel and fieldwork are core parts of the role. Requires a Qualified Mental Health Associate with a Bachelor's degree; CADC I in progress is a plus.

Qualifications

• Minimum two years of experience in social work or related field.
• Spanish language abilities are desirable.
• Experience with trauma-informed care and DEI principles is preferred.

Responsibilities

• Provide community-based interventions and case management to stabilize high-need individuals.
• Collaborate with Mental Health Clinician to implement service plans and coordinate care.
• Coordinate resources, transportation, and scheduling to support housing stability.
• Document all contacts and outcomes per policy within required timelines.
• Engage participants using trauma-informed, culturally responsive approaches.
