Job summary

Your Health is seeking a Clinical Scheduler to coordinate all in-person and virtual encounters for an interdisciplinary care team in the Albany office. This full-time, salaried role ensures patients receive the right number and type of visits under our proactive care model.

The Scheduler will work closely with Directors of Population Health, providers, facilitators, and care divisions to translate clinical recommendations into clear patient visit plans and to optimize scheduling across

Qualifications

• High school diploma or equivalent; healthcare degree preferred.
• Preferred CMA, LPN or RN.
• Experience in a medical or healthcare setting, scheduling or administrative role.
• Familiar with medical terminology, procedures, and insurance verification.
• Strong organizational and time management skills; ability to multitask and prioritize responsibilities.
• Excellent communication and interpersonal skills for all organizational levels.
• Strong problem-solving abilities.
• Ability to work effectively under pressure.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track and follow up with new patients until scheduled.
• Schedule appointments based on care model, availability, urgency, and patient preferences.
• Optimize schedules across all disciplines.
• Follow patients after discharge from inpatient facilities.
• Schedule timely TCM appointments within required windows.
• Maintain thorough documentation.
• Utilize multiple software systems to improve patient follow-up and outcomes.
• Interact with patients via email, portal, phone, and in person to gather information and provide pre-appointment instructions.
