Job summary

Your Health Organization in Savannah, GA is seeking a Clinical Scheduler to support the Care Manager role and coordinate all in-person scheduling for the care team. The position is full-time, 8:00 AM–5:00 PM, Monday–Friday, based in the Savannah office, utilizing multiple software systems to track appointments and patient follow-up.

You will work closely with DPH, providers, facilitators and population health divisions to ensure timely visits and proactive scheduling under our value-based care

Qualifications

• High school diploma or equivalent required; healthcare degree preferred.
• Experience in a medical/healthcare setting, especially scheduling/administrative roles.
• Familiar with medical terminology and insurance verification.

Responsibilities

• Register, verify and schedule patients according to the care model.
• Track and follow up with new patients until scheduled.
• Schedule appointments per care model considering urgency and patient preferences.
• Coordinate schedules across all disciplines and post-discharge follow-up.
• Use multiple software systems to manage patient follow-up and scheduling.
• Communicate with patients via email, portal, phone and in-person.
