Your Health seeks a Clinical Scheduler to support its Care Manager role, coordinating all in-person and provider encounters for patients assigned to a single interdisciplinary care team in Myrtle Beach. The Scheduler ensures the right number and type of visits under our value-based care model, emphasizing prevention, monitoring, and care coordination.

Qualifications

• High school diploma or equivalent; healthcare degree preferred.
• Experience in medical/healthcare scheduling or administrative role advantageous.
• Familiarity with medical terminology and insurance verification.

Responsibilities

• Register, verify and timely schedule patients according to the care model.
• Track new patients and follow until scheduled.
• Schedule appointments per care model including follow-ups based on availability and urgency.
• Optimize schedules across disciplines.
• Coordinate post-discharge scheduling when applicable.
• Document scheduling activities thoroughly.
• Utilize multiple software/tracking systems to monitor follow-up and outcomes.
• Communicate with patients via email, portal, phone, and in person to gather information and confirm appointments.
