Western Reserve Area Agency on Aging in North Olmsted, OH, seeks a Supervisor to oversee care management teams within the MyCare Ohio program. The role requires licensure as an RN or LSW and a background in home care, geriatrics, or related fields.

Responsibilities include staff supervision, field support, care plan reviews, and quality assurance initiatives. The role emphasizes independent judgment, ethics, and participation in program improvements.

Qualifications

• Must hold an Ohio licensed social worker (LSW) or RN license.
• Five years' experience in home care, geriatrics, or related field.
• At least two years of supervisory or leadership experience.
• Excellent verbal and written communication and interpersonal skills.
• Proficient with Microsoft Windows and basic databases.

Responsabilités

• Supervise care managers and conduct team meetings.
• May accompany care managers on field visits for supervision.
• Facilitate decision making and treatment planning among disciplines.
• Assist with case conferences with other service providers.
• Ensure timely client contacts, home visits, reassessments and disenrollment.
• Review care plans for quality and individualized participant care.
