Careforth, Inc. in Akron, OH is seeking a Registered Nurse (RN) Care Manager or Licensed Social Worker (LSW/LISW) to join our care management team.

The role involves onboarding new families, assessing needs, guiding activation of services, and delivering person-centric plans with our digital platform during regular home visits. Candidates must hold an active RN license or LSW/LISW in Ohio and have 2+ years of eldercare experience.

Competenze

Must hold an active RN license and/or be an LSW/LISW in Ohio.
Minimum two years of clinical eldercare experience.
Ability and flexibility to travel within assigned geographic area; valid driver's license and car insurance.
Experience in case management, care planning, and assessment for eldercare and other complex medical needs.
Ability to work in a fast-paced environment and meet department goals and KPIs.
Self-motivated with a results-driven approach.
Tech savvy with proficiency in common software and web-based applications (e.g. Case Management Systems, MS Office).
Additional state-specific qualifications may be required.

Mansioni

Manage enrollment of prospective families and educate on qualifications for consumer, caregiver, and home; assess eligibility for services.
Guide consumers with applications for Medicaid, waivers, and other state benefits.
Conduct initial in-person clinical assessments and coordinate service activation; ensure forms and procedures are followed.
Assist with orientation training for caregivers to onboard to Careforth’s HIPAA secure platform.
Collaborate to develop each member’s plan of care and review on an ongoing basis.
Provide ongoing coaching to drive best practices, caregiver education, and connections to community resources.
Monitor caregiver notes daily and address changes in consumer condition or environment.
Complete documentation for each on-site visit or encounter.
Monitor health status to ensure needs and programmatic requirements are met; manage incidents.
Collaborate with clinical team, agency and support services, primary care physicians, and other providers to ensure timely delivery of services.
Ensure compliance with regulatory standards and company policies; plan discharges and transitions.
Perform other duties as assigned.
