Job summary

Western Reserve Area Agency on Aging is seeking a Service Coordinator to manage MyCare Ohio waiver services for assigned members in community settings. The role evaluates care data, coordinates assessments, and oversees care plans with health partners.

Qualified candidates hold an Ohio LPN/RN or Social Worker license and have at least one year of relevant experience. Travel within five counties, strong computer skills, and commitment to aging services are expected.

Qualifications

• Must have current licensure as an Ohio Social Worker, LPN, or RN.
• Experience in home health care, medical social work, or behavioral health is required.
• Ability to communicate effectively with diverse populations.

Responsibilities

• Evaluate assessment and care management data to determine ongoing care needs and service options.
• Complete MyCare Ohio assessments and enrollment processes as applicable.
• Develop and implement waiver service care plans for members and AL enrollees.
• Coordinate with Medicaid eligibility and monitor service quality and program eligibility.
• Provide education and support to consumers and caregivers.
• Travel to community locations within five-county service areas as needed.
