The Care Management Assistant at Bon Secours (St. Francis Eastside) is a full-time role supporting care management staff with discharge planning and patient transitions.

The ideal candidate will bring strong people skills and a passion for helping others while working in a compassionate, patient-centered environment. Key duties include issuing Medicare messages, collaborating with care managers and social workers, updating on approvals/denials, and maintaining community resource manuals to

Qualifications

• High school diploma or GED is required.
• BLS Basic Life Support certification is required.
• Some college credits are preferred.
• Previous healthcare discharge planning experience is preferred.

Responsabilités

• Assists in issuing Medicare Important Message letters and explains observation status to patients within 24–36 hours of admission.
• Collaborates with Care Managers and Social Workers; handles faxing, mail, phone messages and other office tasks.
• Provides updates on approvals and denials to Care Managers and Social Workers as information is received.
• Assists with non-complex discharge planning and transitions under supervision.
• Keeps community resource manuals up to date.
