Qualifications

• Bachelor's Degree in Nursing required.

Responsibilities

• Facilitates communication among patient/family and multidisciplinary teams to resolve barriers to care.
• Collaborates with care teams to ensure quality, efficient, and cost-effective services.
• Uses evidence-based approaches to engage patients and families in self-management goals.
• Identifies barriers to learning and reports abnormal findings with a plan.
• Manages CM caseload and ensures timely documentation for regulatory requirements.
• Coordinates transitions of care and referrals to internal and external resources.
• Demonstrates age-appropriate care knowledge and adapts to patient needs.
