PrideGlobal is seeking an experienced Registered Nurse to join our Care Management team in Phoenix, AZ. This full-time role coordinates care, referrals, and advocacy across the healthcare journey, with hands-on and virtual member interactions.

The ideal candidate holds an active RN license, CPR certification, Case/Care Management experience, and EMR/EHR experience, plus excellent interpersonal skills to support members, improve outcomes, and navigate the health system.

Formación

• Current RN license in the state of practice.
• CPR certification required.
• Case Management or Care Management experience.
• Electronic Medical Records (EMR/EHR) experience.
• Customer service-oriented with strong patient advocacy.
• Excellent interpersonal and communication skills.
• Strong communication across multiple platforms and modalities.

Responsabilidades

• Support members with real-time consultation, education, scheduling, and coordination of appointments.
• Manage the referral process end-to-end, including follow-up and completion of services.
• Explain recommendations clearly and help members navigate the healthcare system.
• Serve as an empathetic advocate for patients and members.
• Support improved health outcomes and opportunities for cost savings.
• Lead patient outreach related to gaps in care and clinical needs.
• Coordinate care for chronic conditions through education and reminders.
• Assist with the management and integration of office processes.
• Coordinate service and scheduling requests for members.
• Provide the Care Team with referral information and status updates.
• Help develop and manage quality improvement programs at the center.
• Build strong relationships with providers and community organizations.
