Advocate Health in North Carolina is seeking a Registered Nurse for Case Management to support patients across the care continuum. This full-time role collaborates with a multidisciplinary team to coordinate services, manage complex cases, and ensure timely, quality care delivery.

You will operate within 40 hours per week, typically Monday–Friday with on-call expectations during weekends/holidays, and must maintain Epic EMR proficiency while navigating contracts and regulatory requirements.

Formación

• RN license required in the state of practice.
• Bachelor's Degree in Nursing or related field.
• Typically 5 years of clinical nursing or 1–2 years in care management.

Responsabilidades

• Facilitates communications among patient/family and multidisciplinary teams to resolve care barriers.
• Collaborates with primary care, hospital care teams, and post-acute managers to ensure quality, cost-effective care.
• Uses evidence-based approaches to increase patient activation and self-management.
• Identifies barriers to learning and care delivery and develops plans.
• Manages CM caseload independently per department expectations.
• Partners with at-risk patients to deliver continuity of care across diagnosis, treatment, and follow-up.
• Coordinates patient information and referrals between facilities, providers, and community resources.
