Qualifications

• Registered Nurse license required in the practicing state.
• Bachelor's degree in Nursing or related field.
• Typically 5 years clinical nursing or 1–2 years care management experience.

Responsibilities

• Coordinates care with patient, family and multidisciplinary team to resolve barriers to timely care.
• Collaborates with primary and hospital care teams to ensure quality, efficient, cost-effective services.
• Uses evidence-based approaches to engage patients and set SMART goals for self-management.
• Identifies barriers to learning and care delivery and reports findings with a plan.
• Manages CM caseload per department expectations and maintains required documentation.
• Partners with at-risk patients across diagnosis, treatment and follow-up for continuity of care.
• Coordinates patient information between facilities, physicians and community resources for smooth transitions.
• Refers patients to internal departments or external resources as needed.
• Demonstrates age-appropriate care knowledge and communication across the lifespan.
