Job summary

Mercy Health seeks an experienced Manager, Care Management to lead day-to-day operations across multiple locations in Ohio. The role focuses on advancement of care planning, LOS management, readmission prevention, and patient transitions in collaboration with physicians and care teams.

Candidates should have a Nursing or Social Work background with at least 3 years in acute hospital care management, strong leadership, data analysis, and budget acumen in a patient‑centered care model.

Qualifications

• Bachelor of Science in Nursing or Social Work is required.
• Master’s degree preferred for BSMH; required for RSFH for certain locations.
• 3 years of recent acute hospital care management experience required.

Responsibilities

• Lead and oversee day-to-day Care Management operations and standardized practices.
• Educate staff and care teams on progression of care and safe transitions.
• Collaborate with clinicians and leadership to optimize utilization and costs.
