Mercy Health – Youngstown, OH is seeking a Manager of Care Management for St. Elizabeth Youngstown Hospital.

The role oversees daily operations, drives standardized care management practices, and partners with clinical leadership to optimize utilization and transitions of care. You will lead a multidisciplinary team, ensure effective discharge planning, and monitor outcomes with data to improve patient care and reduce costs.

Qualifications

• Bachelor of Science Nursing or Social Work (required)
• Masters, Nursing, Social Work, or Healthcare Administration (preferred for BSMH, required for RSFH)
• BLS Basic Life Support – American Heart Association (required)
• RN License in state of practice or Licensed as a Social Worker in state of practice (required)
• Accredited Case Manager Certification (ACM) or CCM or ANCC Nursing Case Management board certification (preferred)
• 3 years of recent acute hospital care management experience (required)

Responsabilités

• Provides leadership and oversees day-to-day operations to Care Management Team.
• Advocates and educates regarding right care, right time at the right place.
• Utilizes systems within the Care Management Department to support an effective care management process.
• Partners with physician leader to optimize Utilization Management Committee.
• Directs daily operations to achieve effective utilization of personnel resources.
• Participates in the development and management of the annual budget.
