Bon Secours Mercy Health seeks a Manager of Care Management to oversee daily operations for the Care Management Department across Tiffin Hospital, Defiance and Willard. The role blends clinical oversight with financial stewardship to improve patient outcomes and value-based care alignment.

You will lead a multidisciplinary team, ensure standardized care transitions, and collaborate with leadership to meet organizational goals. A BSN or SW is required, with a Master’s preferred.

Qualificações

• 3 years of recent acute hospital care management experience (required)
• Bachelor of Science Nursing or Social Work (required)
• RN license in state of practice or SW license (required)

Responsabilidades

• Provides leadership and oversees day-to-day operations for Care Management Team.
• Coordinates with Care Management Leadership to standardize practices and processes.
• Advocates and educates regarding right care and timely transitions.
• Utilizes systems to support proactive care management across the continuum.
• Directs daily operations to optimize utilization of personnel resources.
