Job summary

Mercy is seeking a Care Manager to join an interdisciplinary team. The role involves assessing and coordinating care from admission to discharge to ensure a safe transition after hospitalization.

You will work within Mercy’s care management model to support patient experience and quality outcomes. The position requires a Master’s in Social Work, active LMSW/LCSW licensure in the state, BLS/CPR at hire or soon after, and a preference for Case Management certification.

Qualifications

• Master's degree in Social Work required or has satisfactorily completed all requirements for the MSW but awaiting conferment
• Must have a current LMSW or LCSW license in the state of employment
• BLS (CPR) at hire date, required, or within 90 days of hire
• Certification in Case Management, Preferred
• 0-2 years' experience in acute care hospital setting

• Assess, plans, advocates, and coordinates care from admission to discharge
• Ensure safe transition post hospitalization and maintain patient experience, safety, and quality of care using performance metrics
