Qualifications

• Bachelor's degree in Nursing (BSN) or Master's degree in Social Work
• Minimum of three years of experience in utilization management, case management, discharge planning, or related care coordination roles
• Current and valid Registered Nurse (RN) or Licensed Social Worker (LISW-S) license in the State of Ohio obtained within 3 years of hire
