Qualifications

• High school diploma or equivalent required.
• Must be 18 years or older.
• Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist or a parent/guardian with Medicaid experience or two years of paid experience in similar functions.

Responsabilités

• Conduct regular outreach to members by phone and in person to assess needs, provide support, and monitor progress.
• Connect members and families to healthcare services, housing assistance, food resources, and community programs.
• Assist in developing and updating individualized care plans to support members' health and wellness goals.
• Coordinate communication among healthcare providers, members, and families to ensure continuity of care and follow-up on referrals.
