Job summary

Carolina Outreach, LLC is seeking a Care Management Extender in Wilson, NC, with virtual support for Fayetteville. You will manage high volumes of calls, coordinate care, and help develop Individualized Service Plans.

The role emphasizes rapport building, advocacy, and collaboration with healthcare providers to improve member outcomes. We offer competitive hourly compensation and a supportive, mission-driven environment with opportunities for growth and training in Tailored Care Management.

Qualifications

• A high school diploma or equivalent;Must be 18 years or older.
• Lived experience with behavioral health condition and/or parent/guardian of someone with behavioral health experience.
• Two years of paid experience performing Extender Functions or related Tailored Care Management tasks.
• On hire, complete general onboarding training;Within 60 days, NC DHHS Tailored Care Management training requirements.

Responsibilities

• Conduct regular outreach to members by phone and in person to assess needs and monitor progress.
• Connect members and families to healthcare services, housing, food resources, and community programs.
• Assist in developing and updating individualized care plans to support health and wellness goals.
• Coordinate communication among healthcare providers, members, and families to ensure continuity of care.
