Job summary

RHA Health Services, LLC is hiring for a Care Manager Extender for Wake/Johnston & Surrounding Counties in Burlington, NC. The role delivers whole-person, integrated care management in sites, homes and communities through face-to-face and team-based work, focusing on physical and behavioral health, I/DD, and related services.

You will promote high-quality care, ensure policy compliance, and collaborate with cross-functional teams to support individuals in the Tailored Care Management program.

Qualifications

• Must have lived experience navigating NC Medicaid delivery system or related health services.
• Certified Peer Support Specialist or equivalent experience preferred.
• Parent/guardian experience navigating Medicaid with related conditions.
• Two years of paid experience with Tailored Care Management eligible population.

Responsibilities

• Care Management service delivery: outreach, engagement and follow-up with members.
• Coordinate services/appointments and transportation reminders.
• Promote health, share information with care team, manage referrals.
• Participate in case conferences and address unmet health needs.
• Assist in data-driven care planning and ensure policy compliance.
• Collaborate with Integrated Care Team, community partners and CIN.
• Maintain professional image and adhere to safety policies.
