Job summary

CVS Health is seeking a Case Management Coordinator in Ohio, combining field-based and telework elements. The role requires travel within the state to member homes and locations, with a typical schedule Monday–Friday, 8–5, and potential after-hours work to meet member needs.

Qualified candidates will have at least 2 years in behavioral health or related fields, a relevant degree, and strong MS Office skills. A comprehensive benefits package and pay range are offered.

Qualifications

• 2+ years experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite applications.
• 2+ years of experience in child mental health, child welfare, or related fields.

Responsibilities

• Coordinate and inform the case management process to facilitate appropriate healthcare outcomes for members.
• Provide care coordination, support and education for members.
• Monitor care plan progress and document activities per regulatory guidelines.

Education

Bachelor's degree or non-licensed master level clinician
