Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers and MS Office Suite (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health or related public sector field providing community-based services.
• 2+ years in family systems, case management, or related areas.
• 2+ years with Ohio delivery systems.
• Willing to travel within Ohio up to 50% and occasional travel to New Albany.
• Reliable transportation and ability to work beyond core hours when needed.
• Bachelor's degree or non-licensed master level clinician in behavioral health or public health.

Responsibilities

• Be clinically and culturally competent in managing complex cases across child-serving systems.
• Evaluate member needs and coordinate care plans with appropriate referrals.
• Collaborate with multidisciplinary teams to optimize health outcomes.
• Coordinate and monitor care plan progress and address barriers to care.
• Provide coaching and support to empower members in healthcare decisions.
