Job summary

CVS Health Corporation is seeking a Case Management Coordinator for a full-time field-based telework role in Ohio. The position requires travel within the Northeast Ohio region up to 50% and prefers candidates from Cuyahoga, Lorain, Medina, Summit, Lake, or Geauga counties.

Typical hours are Monday–Friday 8–5 with some flexibility to meet member needs. The role focuses on coordinating care for children and families, collaborating with MCOs, vendors, and ODM to support complex behavioral health

Qualifications

• 2+ years in behavioral health or human services.
• Experience with MS Office Suite tools required.
• Experience with Ohio delivery systems preferred.
• Travel willingness within Northeast Ohio up to 50%.

Responsibilities

• Coordinate and implement care plan activities and monitor progress.
• Evaluate member needs, eligibility, and plan appropriate interventions.
• Collaborate with medical directors and case managers to optimize outcomes.
• Provide coaching and support to empower patient engagement in care decisions.
• Serve as a single point of contact and address care gaps.
