Job summary

CVS Health in Ohio is seeking a Case Management Coordinator for a full-time, field-based telework position. You will travel up to 50% to member homes and facilities within Central/Southeast Ohio, coordinating care across child-serving systems.

Required qualifications include a bachelor's or non-licensed master’s in behavioral health, 2+ years in related fields, MS Office proficiency, and Ohio delivery-system experience. Travel, flexibility, and reliable transportation are essential.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services/behavioral health care field.
• 2+ years experience in family systems, community systems, case management or related areas.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing and able to travel within the region up to 50% of the time.
• Reliable transportation; mileage reimbursed.
• Willing to work beyond core hours as needed.

Responsibilities

• Be clinically and culturally competent/responsive to manage complex community-based cases across child-serving systems.
• Evaluates referred members’ needs/eligibility and coordinates care plan activities.
• Collaborates with health providers and family teams to optimize outcomes.
• Monitors progress and ensures appropriate care planning and referrals.
• Advocates for quality of care and resolves barriers to services.
• Maintains documentation in line with regulatory and internal policies.

Education

Bachelor's degree or non-licensed master level clinician

2+ years in behavioral health/social/human services

2+ years with MS Office Suite (Outlook/Word/Excel/SharePoint)

2+ years in children's mental health/child welfare/related field

2+ years experience in family systems/child development

2+ years experience with Ohio delivery systems

Willingness to travel up to 50%

Reliable transportation
