• 2+ years of experience in children’s mental health, child welfare, developmental disabilities, juvenile justice, or a public sector human services or behavioral health care field.

Aufgaben

• Provide care coordination, support and education to members through case management tools.
• Collaborate with medical directors, case managers, and health/program staff to optimize outcomes.
• Serve as a single point of contact for members and help remediate gaps in care and access.
• Support development and monitoring of care plans and documentation in compliance with regulations.
