Job summary

CVS Health Corporation in Ohio seeks a dedicated Case Management Coordinator to support members with care coordination, education, and resource navigation.

This is a full-time, field-based telework role requiring travel across Northeast/Northwest Ohio up to 50% and flexible hours to meet member needs. Candidates should reside in the specified counties and bring a background in behavioral health and care coordination.

Qualifications

• Bachelor’s degree or non-licensed master level clinician.
• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or related field.
• 2+ years experience in family systems, community resources, case management, or related areas.
• 2+ years experience with Ohio delivery systems and local networks.
• Willing and able to travel within the assigned region up to 50% of the time; reliable transportation required.

Responsibilities

• Manage complex cases in the community across child-serving systems.
• Evaluate member needs/eligibility and coordinate care plans with teams.
• Coordinate and implement care plan activities and monitor progress.
• Consult with case managers and Medical Directors to overcome barriers.
• Collaborate with Child and Family Teams and escalate quality issues.
• Use motivational interviewing to promote engagement and healthy choices.
• Serve as single point of contact for members and assist with gaps in care.
