Job summary

CVS Health Corporation is seeking a Case Management Coordinator in Ohio to provide care coordination for members in a behavioral health program. The role requires evaluating needs, coordinating care plans, and collaborating with multidisciplinary teams while traveling within Northeast Ohio up to 50% of the time.

The position demands experience in behavioral health, child services, and case management, with a Bachelor's degree or higher and willingness to work beyond standard hours.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years of experience with personal computers, keyboard and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
• 2+ years of experience in children's mental health, child welfare, developmental disabilities, juvenile justice, or a public sector field.
• 2+ years experience in family systems, case management, child and family counseling/therapy, child protection, or development.
• 2+ years experience with Ohio delivery systems, including local networks/resources.
• Willing and able to travel within the assigned region up to 50% of the time; Some travel to the New Albany office may be required.
• Reliable transportation
• Willing and able to work beyond core business hours, as needed.

Responsibilities

• Conduct comprehensive evaluation of referred member's needs/eligibility and recommend approach to care resolution.
• Coordinate and implement assigned care plan activities and monitor progress.
• Consult with case managers, supervisors, Medical Directors to overcome barriers and achieve outcomes.
• Work with Child and Family Teams; escalate quality of care issues as needed.
• Utilize case management processes in compliance with guidelines and company policies.
