CVS Health in Ohio is seeking a Case Management Coordinator for a full-time, field-based telework role. You will travel up to 50% within Northeast/Northwest Ohio to member homes, coordinating care and education to support families.

A bachelor's degree or non-licensed master level clinician is required, with 2+ years in behavioral health and child services, plus strong MS Office skills and willingness to travel.

Qualifications

• Bachelor's degree or non-licensed master level clinician required.
• 2+ years in behavioral health, social services, or human services.
• 2+ years using MS Office Suite applications.
• 2+ years in children's mental health or related public sector field.
• 2+ years experience in family systems, case management or care coordination.
• 2+ years experience with Ohio delivery systems.
• Willing to travel within Ohio up to 50% of the time.

Responsibilities

• Coordinate and inform case management processes to support healthcare outcomes.
• Evaluate referred members and monitor care plans and progress.
• Collaborate with teams to overcome barriers and ensure quality of care.
• Serve as a single point of contact and assist members to remediate gaps in care.
