Job summary

CVS Health Corporation is recruiting for a full-time field-based and telework Case Management Coordinator in Ohio. The role requires traveling within the state up to 50% to member homes and locations, with a Monday–Friday 8–5 schedule and flexibility for after-hours coverage.

The position supports the OhioRISE program, coordinating care for children with complex behavioral health needs across systems, and promoting high-quality care through collaboration with care teams.

Qualifications

• 2+ years of experience in behavioral health, social services, or human services.
• 2+ years with MS Office applications (Outlook, Word, Excel, SharePoint).
• 2+ years in children's mental health or related fields.
• 2+ years in family systems, case management, or child development.
• 2+ years experience with Ohio delivery systems.
• Willing to travel within the assigned region up to 50% of the time.
• Reliable transportation.
• Willing and able to work beyond core hours when needed.
• Bachelor's degree or non-licensed master level clinician.
• Must reside in Ohio.

Responsibilities

• Collaborate to facilitate healthcare outcomes via care coordination and education for members.
• Evaluate referred members' needs/eligibility and plan care.
• Monitor and implement care plan progress.
• Coordinate with Child and Family Teams and other stakeholders.
• Identify and escalate quality of care issues.
• Provide coaching and support to empower members in decision-making.
