Job summary

The Buckeye Ranch is seeking a Care Management Coordinator to join our Care Management Entity. The role focuses on engaging children, youth, and families with behavioral health needs, delivering care coordination using High Fidelity Wraparound, and working with community partners to connect families with services.

We value trauma-informed, culturally responsive, and family-centered care. The CME operates under OhioRISE guidelines, advocating for coordinated care and strong community support

Qualifications

• Bachelor's degree in human services preferred.
• Experience in children's behavioral health or related public sector human services.
• Active driver’s license required.

Responsibilities

• Complete training in High Fidelity Wraparound.
• Identify care coordination needs and support care plans.
• Schedule and coordinate family contact across home, school, and community.
• Ensure services align with care plans and safety needs.
• Link families to local services and resources.
• Maintain documentation including assessments and care plans.
