Job summary

The Buckeye Ranch is seeking a Care Management Coordinator to join the Care Management Entity. This role engages with children, youth, and families in the community to coordinate care using High Fidelity Wraparound principles.

Responsibilities include care planning, coordinating family-based services, and documenting assessments and plans. A background in human services and a valid driver’s license are preferred, with OhioRISE CME involvement noted.

Qualifications

• Bachelor's Degree in human services field is preferred.
• Experience in children’s behavioral health or related public sector human services.
• Active driver’s license and auto insurance.

Responsibilities

• Complete training in High Fidelity Wraparound and ICC/MCC cases.
• Identify care coordination needs and support care plans.
• Schedule and coordinate family visits based on needs.
• Ensure services meet safety and treatment needs.
• Link families to local community services and resources.
• Develop collaborative relationships with partners and community resources.
• Complete documentation including assessments and care plans.
