Integrated Services for Behavioral Health in Ohio seeks a Care Coordinator to link services for children, youth, and families with behavioral health needs. The role emphasizes coordination across home, school, and community and building relationships with community partners to support care plans.

The position requires knowledge of wraparound services, cultural humility, and strong organizational skills; driver’s license and ability to conduct virtual meetings are required.

Qualifications

• Experience providing services to children and families in behavioral health or related public services.
• Hi-Fidelity Wraparound certification provided at time of employment.
• Two years in a coordinated supportive services or care coordination role preferred.
• Experience with autism spectrum disorders and developmental disabilities preferred.
• Cross-systems care coordination and culturally humble approach requested.
• Excellent organizational skills and ability to manage multiple tasks.
• Valid driver’s license required; able to conduct video calls/meetings with reliable internet.

Responsibilities

• Identify care coordination needs with families to align with service delivery standards.
• Define cultural factors influencing strengths and family interaction styles.
• Coordinate family-based services in home, school, and community.
• Ensure services meet safety and treatment needs of child, youth, and family.
• Engage with child and family teams to support integrated care plans.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation and participate in program evaluation and planning meetings.
• Remain current with required trainings including High Fidelity Wraparound and Cultural Humility.
