Job summary

National Youth Advocate Program (NYAP) is seeking a full-time CME Care Coordinator Supervisor to lead a team of Care Coordinators delivering wraparound care coordination for OhioRISE youth with moderate behavioral health needs across Catchment Area C. The aim is to keep youth in their homes and communities by coordinating services and reducing out-of-home placements.

Qualifications

• Minimum Qualifications include: licensed or unlicensed practitioner as per rules, relevant community-based services experience with children and families, and appropriate education paths.
• Supervisors of care coordinators require high-fidelity wraparound training and access to supervision.
• Proficiency with computers and software, and cultural competence for complex cases.
• Live in one of the Catchment Area C counties.

Responsibilities

• Recruit, interview and train potential new Care Coordinators.
• Ensure monthly and yearly goals for staff and program.
• Provide formal and informal staff training, ongoing and individualized.
• Conduct performance evaluations and develop corrective action plans.
• Provide regular supervision with staff and feedback.
• Facilitate staff meetings and ensure holistic family treatment.
• Review program paperwork for quality and treatment planning.
• Ensure adequate staff and crisis coverage for clients.
