Job summary

Positive Education Program seeks a qualified professional to conduct comprehensive assessments and provide wraparound care coordination (ICC/MCC) through the Ohio RISE CME. You will collaborate with families, clinicians, and community resources to create and implement child- and family-centered plans.

The role emphasizes trauma-informed practice, coordination of services, and maintaining documentation while supporting youth across care settings.

Qualifications

• Bachelor's degree required in social work, sociology, psychology or related field; three years' experience in child behavioral health acceptable in lieu of degree
• Minimum of two years' experience in children's behavioral health or related field providing community-based services to children and families
• Master's degree with 1 year of experience in children's behavioral health preferred
• Background in family systems, community resources, case management, or child development
• Culturally competent or responsive with experience managing complex cases
• Experience working with children and families experiencing SED, trauma, or co-occurring disorders
• Strong written and oral communication; good judgment and decision-making
• Ability to work professionally and collaboratively with others

Responsibilities

• Align with agency mission and trauma-informed Sanctuary Model.
• Provide structured care planning and high fidelity wraparound services (ICC/MCC).
• Meet OhioRISE care coordination rules for ICC or MCC.
• Conduct initial face-to-face contact and update comprehensive CANS assessments.
• Facilitate child and family team meetings and develop care plans.
• Develop crisis safety plans for the client and family.
• Make referrals and linkages to services and natural supports.
• Facilitate discharge and transition planning across settings.
• Collaborate with team members and families to achieve outcomes.
• Maintain required paperwork and compliance with policies.
• Transport clients when needed and ensure safety in travel.
