Red Rock Behavioral Health Services is seeking a Care Coordinator to serve as a single point of accountability for access, coordination, and delivery of behavioral health services. This role supports Wraparound and Service Coordination within the Health Team, ensuring culturally and linguistically relevant care.

You will work with diverse populations, complete required documentation on time, and connect families with community resources while participating in ongoing trainings and team meetings.

Qualifications

• BHCM II certification or equivalent behavioral health credential.
• Ability to work with diverse populations and communicate effectively.
• Familiarity with wraparound and service coordination is a plus.

Responsibilities

• Coordinate medically necessary services across mental and physical health.
• Complete multi-level assessments within required timelines.
• Maintain relationships with community teams and resources.
