Job summary

Jobtailor seeks a dedicated care coordination specialist to support children and families in Ohio, focusing on behavioral health and developmental disabilities. You will plan and lead family-centered meetings, coordinate services at home or in the community, and partner with youth, families, natural supports, and providers to identify needs and goals.

Responsibilities include documentation in EHRs, compliance with HIPAA, and travel within the service area.

Qualifications

• Experience with care coordination in behavioral health or related public services.
• Knowledge of Hi-Fidelity Wraparound preferred; certification provided at employment.
• Experience with children and families in systems like Medicaid, school, and community services.
• Strong verbal and written communication; ability to maintain confidentiality.

Responsibilities

• Create, plan, organize, lead, and facilitate child and family team meetings.
• Provide care coordination in the family’s home, community, or preferred location.
• Partner with youth, families, natural supports, and service providers to identify needs and services.
• Support youth at home, in school, and in their communities.
• Lead teams in identifying needs, goals, and action steps.
• Maintain documentation required to access and maintain OhioRISE benefits.
• Travel within the assigned service area.
• Perform other duties as assigned.
