Cadence Care Network seeks an OhioRISE Care Coordinator to focus on family-based, responsive services using the High-Fidelity Wraparound model. This role involves developing Child and Family Teams, leading team meetings, and ensuring care plans conform to Ohio Department of Medicaid standards.

Candidates must have experience in children's behavioral health or related fields and meet educational and licensing requirements.

Qualifikationen

• Experience in children’s behavioral health or related human services field.
• Valid Driver License, Auto Insurance and provision of a vehicle for work.

Aufgaben

• Cultivate family-focused, community-based responsive services.
• Develop and maintain the Child and Family Team.
• Coordinate implementation of the Plan of Care.

Ausbildung

High School Diploma plus 3 years’ experience

Associate or bachelor’s degree plus 2 years’ experience

Master’s degree plus 1 year experience
