The Buckeye Ranch is seeking a Care Management Coordinator to join our Care Management Entity. The Care Management Coordinator is skilled at engaging and working with children, youth, and families in the community with significant behavioral health needs. Care Coordination team members provide care coordination activities aligned with The Buckeye Ranch (TBR) philosophy and mission – Providing hope and healing through family-centered, strengths based, trauma-informed, culturally responsive, and integrated and coordinated care. Care Coordination team members have a thorough understanding of local communities, skilled at developing working relationships with community agencies, and can identify potential community support for development to assist families/caregivers working collaboratively with Child and Family Teams. Care Coordination staff ensure children, youth and families have a voice and choice in all coordinated care and services provided pertinent to High Fidelity Wraparound certification training.

The Buckeye Ranch is thrilled to support the mission of OhioRISE as a lead Care Management Entity in East Franklin County! OhioRISE (Resilience through Integrated Systems and Excellence) is a specialized managed care program for youth with complex behavioral health and multi-system needs. As a Care Management Entity, The Buckeye Ranch will advocate and build a system of care, will provide Care Coordination with focus on meeting the unique needs of children and families, ensuring the best care, the right care, supported by strong communities. With partners across the community, The Buckeye Ranch CME will be a central place to help families, providers, and community partners navigate our complex systems. The work of the CME is in line with the mission of The Buckeye Ranch, restoring hope and providing healing for children, youth, and families.
