Grace at Work in Bedford, IN is seeking a compassionate Intake Coordinator to join our team. You’ll be the first point of contact for clients and families, guiding them to care and support services with empathy.

You will manage the intake from inquiry to enrollment, conduct screenings, keep records up to date, and coordinate with internal care teams to ensure smooth transitions. This role requires organization, strong communication, and a client-first approach.

Qualifications

• High school diploma or GED required; associate's or bachelor's degree in human services or related field a plus.
• Knowledge of HIPAA and client confidentiality requirements would be beneficial.
• Experience in intake coordination, case management, social services, or related field preferred.

Responsibilities

• Manage the client intake process from initial inquiry through enrollment and onboarding.
• Conduct intake screenings, assessments, and interviews with prospective clients and families.
• Maintain accurate and up-to-date client records, documentation, and intake forms.
• Coordinate with internal care teams to ensure smooth client transitions and service placement.
• Respond promptly to phone, email, and in-person inquiries with professionalism and empathy.
• Track intake data and generate reports to support organizational goals.
• Ensure compliance with applicable regulations, policies, and confidentiality standards.
